If you are a NEW PATIENT: in order to prevent cancelation or delay of appointment, there are a few quick steps to complete at least 48 hours before your appointment.

  • 1️⃣ Complete Your Paperwork

    • Check your email for a link to your intake forms.

    • Forms must be submitted at least 48 hours prior to keep your appointment.

  • 2️⃣ Create Your Patient Portal

    • Log in or create your account using the link in your email or https://www.optimantra.com/optimus/om/patient/login

    • Your portal lets you:
      ✅ Access appointment details / schedule appointments
      ✅ Send secure messages
      ✅ View and upload documents

    • Click the “NEW SIGN UP“ option to set a password using your email

  • 3️⃣ Upload Required Documents

    • Driver’s License / Photo ID

    • Insurance Card (front & back)

    • Upload directly through your patient portal or by replying to your intake email if you haven’t already sent it to us.

  • 4️⃣ Check Your Spam Folder

    Sometimes our emails take the scenic route!

    • Look for messages from: info@truenorthpsychiatry.com and no-reply@optimantra.com

    • Mark them as “Not Spam” so you don’t miss important updates.
