If you are a NEW PATIENT: in order to prevent cancelation or delay of appointment, there are a few quick steps to complete at least 48 hours before your appointment.
1️⃣ Complete Your Paperwork
Check your email for a link to your intake forms.
Forms must be submitted at least 48 hours prior to keep your appointment.
2️⃣ Create Your Patient Portal
Log in or create your account using the link in your email or https://www.optimantra.com/optimus/om/patient/login
Your portal lets you:
✅ Access appointment details / schedule appointments
✅ Send secure messages
✅ View and upload documentsClick the “NEW SIGN UP“ option to set a password using your email
3️⃣ Upload Required Documents
Driver’s License / Photo ID
Insurance Card (front & back)
Upload directly through your patient portal or by replying to your intake email if you haven’t already sent it to us.
4️⃣ Check Your Spam Folder
Sometimes our emails take the scenic route!
Look for messages from: info@truenorthpsychiatry.com and no-reply@optimantra.com
Mark them as “Not Spam” so you don’t miss important updates.
