If you are a NEW PATIENT: To confirm your appointment, please complete all required paperwork and consent forms within 24 hours of your request. Incomplete or missing forms may result in cancellation so we can accommodate other patients in need of care.

  • 1️⃣ Complete Your Paperwork

    • Check your email for a link to your intake forms.

    • Forms must be submitted within 24 hours to confirm your appointment.

  • 2️⃣ Create Your Patient Portal

    • Log in or create your account using the link in your email or https://www.optimantra.com/optimus/om/patient/login

    • Your portal lets you:
      ✅ Access appointment details / schedule appointments
      ✅ Send secure messages
      ✅ View and upload documents

    • Click the “NEW SIGN UP“ option to set a password using your email

  • 3️⃣ Upload Required Documents

    • Driver’s License / Photo ID

    • Insurance Card (front & back)

    • Upload directly through your patient portal or by replying to your intake email if you haven’t already sent it to us.

  • 4️⃣ Check Your Spam Folder

    Sometimes our emails take the scenic route!

    • Look for messages from: info@truenorthpsychiatry.com and no-reply@optimantra.com

    • Mark them as “Not Spam” so you don’t miss important updates.
